Today while out on a hack (trail ride), my mare suddenly started head-shaking hysterically, nearly shaking her bridle off. She was beside herself for a while, and I assume that a bug of some kind had got into her ear.

Although she calmed down eventually, I found that back at the barn she was still hyper-sensitive when I tried to examine her ears, swinging her head away when I came anywhere near them (very unusual as she normally loves having her head handled) although not head-shaking any more if left alone.

When we finally managed to peer into her ears, we couldn't see anything wrong. I'm hoping that if an insect bit her, it then flew out, but if it's still in there, presumably dead, how bad is that? Should I ask the vet to have a look just in case, or assume that if she's back to normal tomorrow, all is well?

Thanks for your time.

If the head shaking is gone, what ever caused it is probably gone too and I cannot think of any emergency that might need immediate attention if there are no other symptoms than some ear shyness. You may have to retrain the ear sensitivity problem how ever.
