I have a percheron gelding that perpetually hangs his penis out incouraging flies to land there and has an infected penis tip. He had his sheath cleaned about 3 months ago, and since then the tip where the urine comes out has become increasingly red, irritated and bloody. We had a vet out for colic and she looked and said to put SWAT on it. However we recently found a large unhealthy scab (possibly from the flies) on the tip which was restricting urination. We picked it off and it created a very large and bloody crater. We then put triple antibiotic on it but need to know how else to treat it and what causes this.

Hello Kathy,
Hmmm it is not uncommon to see mild urethritis at the tip of the penis. The tip of the urethra is reddened and there may be a very mild erosion. You see this so commonly in fact I often note it and ask folks to watch it and it is rarely progressive or causes a problem. The one exception is when you see remarkable ulceration of nodules in which case squamous cell carcinoma needs to be considered and considered early because of the potential of metastasis. Culture and biopsy (impression smear, scrapping, or actual biopsy) would be the next course for a significant urethral lesion.
