Work on the diagnostic paradigm has really slowed with this topic. We have spent over a month working on this update and its related articles. The problem is to try and approach swellings in a logical manner without an overwhelming amount of detail so that you can use the article to help tell why and what is swelling. I hope we achieved our ends and feed back appreciated.
